This form is used to collect the essential information needed to add a new organization or facility to the Rovicare platform. By completing it, we ensure the provider is correctly configured in the system, visible to referral partners, and able to receive referrals securely. The form captures details for both operational setup and compliance requirements.
Note: You will be responsible for adding this organization to your "Preferred Providers" after it has been added to the system.
Update Existing Organization Information:
This form is used to capture changes or corrections to the details of an existing organization listed on the Rovicare platform. Keeping provider information current ensures referrals are sent accurately, and care partners always have the right contact and service details.
